If you’re considering Viagra or its generic version, knowing the real sildenafil side effects matters more than the marketing copy on the pharmacy bag suggests. Most men do fine on it. A meaningful minority don’t, and a small group runs into problems serious enough to land them in an ER.
Sildenafil has been FDA-approved since 1998 and remains one of the most-studied erectile dysfunction medications in the world. That long track record is reassuring, but it doesn’t mean every patient sails through. What I tell patients is this: knowing the seven side effects below ahead of time means you’ll recognize them quickly, react sensibly, and avoid the panicked 2 a.m. Google search.
Here’s the honest rundown.
How Sildenafil Works (And Why the Side Effects Make Sense)
Sildenafil is a PDE5 inhibitor. It blocks the enzyme that breaks down a chemical called cGMP, which lets blood vessels in the penis relax and fill. The catch is that PDE5 isn’t only found there. It shows up in vessels throughout your body, which is why most sildenafil side effects involve blood flow somewhere it wasn’t strictly intended.
That single mechanism explains roughly 80% of what you’re about to read. Once you understand it, the side effect list stops feeling random.
1. Headache
The most common one. In clinical trials, around 16% of men report headache at standard doses, making it the runaway leader on the side effect list. It’s caused by vasodilation in the cranial vessels and tends to feel like a dull pressure rather than a migraine.
Most patients find acetaminophen handles it. If headaches are severe or recurrent, a lower dose often fixes the problem. 2. Facial Flushing

5. Vision Changes
This one gets the most patient questions. Some men notice a faint bluish tint to their vision, increased light sensitivity, or blurriness lasting an hour or two. It happens because sildenafil weakly inhibits PDE6, an enzyme in the retina.
These transient sildenafil side effects are well-documented and almost always harmless. What is NOT harmless, and you should treat as an emergency, is sudden vision loss in one or both eyes. That can signal non-arteritic anterior ischemic optic neuropathy (NAION), a rare but serious complication. Stop the drug and seek care immediately.
6. Dangerous Drop in Blood Pressure (With Nitrates)
This is the side effect that can actually kill you. Sildenafil combined with nitrates, including nitroglycerin tablets, isosorbide, and recreational "poppers", can cause blood pressure to crash to fatal levels.

- Chest pain or pressure during or after sex
- Sudden vision loss, hearing loss, or ringing in the ears
- Erection lasting longer than four hours
- Fainting or severe lightheadedness
- Skin rash, swelling of the lips or tongue (rare allergic reaction)
