If you’ve been handed a prescription for arthritis pain, the celecoxib vs meloxicam question probably came up at some point. Maybe your doctor mentioned both. Maybe your insurance pushed one over the other. Either way, you want to know which is actually safer for your stomach, your heart, and your kidneys over the long haul.
Both drugs are NSAIDs (nonsteroidal anti-inflammatory drugs), and both are taken daily by millions of Americans with osteoarthritis or rheumatoid arthritis. But they work slightly differently, and that difference matters more than most patients realize.
The short answer? Neither is universally "safer." The safer drug depends on which organ system you’re most worried about. According to the FDA’s NSAID labeling guidance, all NSAIDs carry cardiovascular and gastrointestinal risks, but the magnitude varies meaningfully between agents.
How Celecoxib and Meloxicam Actually Work
Both drugs reduce pain and inflammation by blocking COX enzymes, which produce prostaglandins. The catch: there are two COX enzymes, and they do very different things.
COX-1 protects your stomach lining and helps platelets clot. COX-2 drives inflammation. Older NSAIDs like ibuprofen and naproxen block both, which is why they cause ulcers.
Celecoxib (brand name Celebrex) is a selective COX-2 inhibitor. It targets inflammation while mostly leaving the stomach-protective COX-1 alone. Meloxicam (Mobic) is what pharmacologists call "preferentially" COX-2 selective. At low doses (around 7.5 mg), it behaves a lot like celecoxib. At higher doses (15 mg), it starts blocking COX-1 too, which changes its safety profile.
That single distinction drives most of what follows in the celecoxib vs meloxicam debate.
Stomach and GI Safety
If your main concern is ulcers, GI bleeding, or stomach irritation, celecoxib generally has the edge. Multiple meta-analyses, including data summarized by the NIH’s MedlinePlus database, show celecoxib produces fewer endoscopically detected ulcers than nonselective NSAIDs.
Meloxicam sits in the middle. At 7.5 mg daily, GI risk is reasonably low. At 15 mg, the risk climbs closer to traditional NSAIDs.
Patients at higher GI risk include:
- Anyone over 65
- People with a prior ulcer or GI bleed
- Those taking aspirin, steroids, or anticoagulants
- Heavy alcohol users
For these patients, what I often see prescribed is celecoxib paired with a PPI like omeprazole. It’s not a guarantee, but it’s the lowest-risk NSAID combination we have for chronic use.
Cardiovascular Risk
Here’s where things get interesting. The PRECISION trial, published in NEJM in 2016, randomized over 24,000 arthritis patients to celecoxib, ibuprofen, or naproxen and followed them for years. Celecoxib was non-inferior to both on cardiovascular events. That surprised a lot of cardiologists who had written off COX-2 inhibitors after Vioxx was pulled in 2004.
Meloxicam wasn’t in PRECISION, so we have less head-to-head data. Observational studies suggest its cardiovascular risk is roughly comparable to celecoxib at low doses and climbs at 15 mg.
If you have established heart disease, neither drug is a slam dunk. Both carry the FDA’s class-wide black box warning for cardiovascular events. The American Heart Association generally recommends using the lowest effective NSAID dose for the shortest possible duration in patients with cardiovascular disease. If you’re already managing multiple cardiac medications, the same caution applies to NSAIDs that applies to combinations like tadalafil with blood pressure meds, interactions matter.
Kidney and Blood Pressure Effects
Both drugs can raise blood pressure and reduce kidney function, especially in patients already on ACE inhibitors, ARBs, or diuretics (the so-called "triple whammy"). In practice, the kidney risk between celecoxib vs meloxicam looks pretty similar.
What I tell patients: if your baseline creatinine is normal and you’re not on multiple blood pressure meds, short-term use is usually fine. If you’re already managing hypertension or chronic kidney disease, get a basic metabolic panel a few weeks after starting either drug.
Onset, Dosing, and Convenience
Celecoxib is typically dosed twice daily for arthritis (your prescriber will determine the right dose based on your condition and other meds). Meloxicam is once daily, which some patients prefer. Both take a few days to a couple of weeks to reach full effect for chronic arthritis, unlike ibuprofen which works within hours.

The honest answer is, it depends on your risk profile.
- If you have GI risk factors (prior ulcer, age over 65, blood thinners): celecoxib is usually the safer choice, ideally with a PPI.
- If you have cardiovascular disease: either is acceptable at the lowest effective dose, with close monitoring.
- If cost is the deciding factor and you have no major risk factors: meloxicam 7.5 mg is a reasonable starting point.
- If you have sulfa allergy: celecoxib contains a sulfonamide group and is generally avoided. Meloxicam wins by default.
