In the summer of 2013, Pfizer’s patent for Viagra, the company’s blockbuster drug for erectile dysfunction (ED) expired in Western Europe, prompting a flood of generic competitors and driving the price of drugs containing sildenafil, the active ingredient in Viagra, down sharply.

The drop in price has increased demand in the UK. The increase in requests for prescriptions for Viagra and its generics first showed up in men who pay to see private physicians, but it is expected to increase among men who receive their healthcare through the National Health Service (NHS), because NHS guidelines on prescribing the drug are expected to be relaxed in the near future.

Prices Plunge

Last June, when Pfizer’s patent for Viagra expired in Europe, numerous manufacturers of generic versions of the drug were standing by, ready to introduce their products. As a result, the prices for drugs containing sildenafil plunged from £21.27 for a pack of four (approximately US$35.48) to £1.45 (US$2.42) per four-pack, a price drop of around 93%. But even when Pfizer’s patents were in effect in all the company’s markets, Viagra was a wildly popular drug, bringing in around $2 billion per year worldwide. It’s still considered the most counterfeited drug in history. The dramatic drop in price in Europe is likely to bring still stronger increases in demand in generics.

Expected Changes in NHS Prescription Guidelines

In the UK, the Department for Health regularly reviews its prescription regulations, and under current guidelines, only men with specific conditions, such as multiple sclerosis, diabetes, and prostate cancer are able to obtain ED medications with an NHS prescription. However, new consultation research by the Department of Health has shown that the NHS should be able to afford to cover Viagra (and its generics), even if demand for the drug increases threefold, due to the lower prices.

Another reason the NHS wants to be able to cover ED drugs for more men is that making the drug readily available through the NHS could discourage men from buying the drug from questionable websites. Yearly crackdowns on counterfeit Viagra peddlers temporarily shut down operations that sell ineffective or dangerous fake versions of the drug, but the market for fakes is lucrative enough that unscrupulous sellers find ways to launch new websites. Counterfeit Viagra has been found to have little to no active ingredient, plus the presence of substances including paint and drywall material. The risk from ordering the drug through un-vetted, offshore websites is significant, both health-wise and legally.

Could Theft Be Less of a Problem Too?

Theft has also been a problem with Viagra in the UK. For example, the Ministry of Defence (MoD) reported that since 2007, approximately £6,000 worth of Viagra had been stolen, along with other items including 100 bayonets, thousands of rounds of live ammunition, copper pipes, electrical cable, a truck, and a washing machine. The drugs were intended for service members with ED related to health conditions like high blood pressure. With Viagra and generic versions more readily available – particularly once NHS prescribing guidelines are relaxed – law enforcement officials hope that the drug will be less of a target for thieves.

Viagra Patent Status in the US

Generics for Viagra won’t be available until 2017, under an agreement between Pfizer and Teva Pharmaceuticals, which worked out an agreement to sell its generic version before Pfizer’s US patent expires in 2020. From the time Teva starts selling its generic until Pfizer’s patent lapses, Teva will be the only generic for Viagra sold in the US. While this limited competition should help depress prices to some extent, a dramatic drop-off in price isn’t expected in the US until other generics are allowed to be sold here too.
