This is a molecular model of sildenafil citrate, the active ingredient in Viagra.

For a drug candidate that early on failed to make the grade as a treatment for angina, Viagra has gone on to make quite a name for itself. It debuted on the world stage in 1998 as an oral medication to treat erectile dysfunction caused by impaired blood flow to the penis, which is said to account for roughly 80 percent of all male impotence. But in the two decades that have followed, Viagra has demonstrated that its medicinal properties extend well beyond the treatment of ED.

Sildenafil citrate, Viagra’s active ingredient, has also been used to treat a wide array of other ailments, including the following:

  • Pulmonary arterial hypertension (PAH): This is a condition characterized by elevated blood pressure in the lungs that diminishes respiratory function. Under the brand name Revatio, Pfizer, which holds the patent on Viagra until April 2020, markets sildenafil citrate as a treatment for PAH. The same vasodilatory properties that allow the medication to optimize blood flow and thus temporarily facilitate near-normal erectile function also help to open up arteries that supply the lungs, thus reducing pulmonary blood pressure.
  • Systolic heart failure: Arnon Blum, M.D., a professor of cardiology at Israel’s Bar Ilan University, authored a study that appeared in the July-August 2009 issue of “Congestive Heart Failure.” In it, Blum wrote that testing had demonstrated that treatment with sildenafil “improves exercise capacity and quality of life in patients with systolic heart failure, especially in those who develop secondary pulmonary hypertension.” Systolic failure occurs when the heart’s left ventricle fails to contract normally.
  • Cancer: A growing number of studies indicate that sildenafil citrate produced significantly improved results when used in combination with certain anticancer agents. In some cases, it’s been suggested that sildenafil improves the immune system’s ability to search out and destroy malignant cells. In a recent article in the online journal ecancermedicalscience, a researcher said that sildenafil and the active ingredients in other oral ED drugs were being studied for their ability to penetrate the blood-brain barrier, which would help treat certain brain cancers.
  • Altitude sickness: Individuals who normally live and work at lower elevations sometimes suffer from altitude sickness when they travel to regions at extremely high elevation. Symptoms range in severity from nausea and profound fatigue to cerebral edema and hyperventilation. Although the evidence here is still somewhat equivocal, some have reported that sildenafil reduces the severity of these symptoms. It has also been shown to improve athletic performance in athletes accustomed to competing at lower elevations.
  • Raynaud’s syndrome: Also known as Raynaud’s phenomenon or Raynaud’s disease, this disorder causes certain parts of the body — fingers and toes most often — to feel cold and numb. A study published in a 2005 issue of “Circulation” concluded that “sildenafil is an effective and well-tolerated treatment in patients with Raynaud’s phenomenon.”

Even as this is written, researchers in laboratories across the world are exploring yet other applications for sildenafil citrate, a surprisingly versatile medication.
