More than 18 years after it first became available in the United States, Viagra remains one of the nation’s top-selling drugs for the treatment of erectile dysfunction. In less than two decades, Pfizer’s groundbreaking drug has dramatically altered the treatment of ED in this country and around the world. It has also paved the way for the introduction of a handful of similar impotence medications.
For at least another year, Viagra is protected from generic competition in this country. Actually, Pfizer’s U.S. patent on the little blue pill doesn’t expire until April 2020, but the drugmaker struck an agreement in 2013 that could permit generic competition as early as December 2017. Under Pfizer’s accord with Israeli pharmaceuticals giant, Teva, the latter could begin to market a generic formulation of Viagra late next year. However, because Pfizer’s patent is valid until 2020, Teva would be required to pay the American drugmaker a royalty for the privilege of manufacturing and marketing a generic version of the drug.
Available Only by Prescription

It All Started with Viagra
As previously noted, Viagra was the first of a new wave of oral erectile dysfunction medications to be marketed in the United States and elsewhere. It was the first of a family of medications known collectively as PDE5 inhibitors, so called for their ability to temporarily deactivate an enzyme known as phosphodiesterase-5, which can interfere with robust blood flow to the penis.
The vast majority of erectile dysfunction is caused by insufficient blood flow to the penis, which can be traced to atherosclerosis, the buildup of fatty plaque on artery walls, and a number of other physiological ills. It’s estimated that 10 to 20 percent of ED is psychological in origin, and Viagra and the other PDE5 inhibitors are unlikely to overcome impotence of this type.
Erections Start in the Brain
Erections begin in the brain and not in the penis. When the body’s nerve center detects feelings of sexual desire or physical stimuli to a man’s erogenous zones, it releases a flood of nitric oxide that courses toward the pelvic region. The nitric oxide triggers secondary chemical reactions, one of which gives rise to a substance known as cyclic guanosine monophosphate, or cGMP. As blood levels of cGMP rise, the smooth muscles lining the arteries that feed the penis relax, increasing the volume of blood they can carry. This rush of blood into the penis quickly fills the spongy erectile tissue of the penis, creating an erection.
One of the primary tasks of the PDE5 enzyme is to break down cGMP. This is of little concern in men with robust vascular health, because more cGMP arises to take its place. In those whose blood flow is already below par, the PDE5 enzyme can quickly wipe out the cGMP and thus sabotage erectile function.
How PDE5 Inhibitors Work
Viagra and the other PDE5 inhibitors hold the PDE5 enzyme at bay for four or more hours, which gives a man time to achieve and maintain an erection strong enough for sexual activity. It is not a cure for ED, but it does allow a man to temporarily overcome the effects of ED so that he can enjoy an active sex life.
Viagra has a few side effects, most of them mild and most of which disappear upon continued use. These include back and muscle pain, dizziness, headache, nausea, runny or stuffy nose, and upset stomach. More serious side effects, which are rare, can include a sudden loss of vision or hearing and an erection that lasts for more than four hours.
