On March 27, 1998, the U.S. Food and Drug Administration gave its official stamp of approval to a little blue, diamond-shaped pill that has gone on to revolutionize the treatment of erectile dysfunction not just in the United States but around the world.
Nearly two decades have passed since Viagra’s debut. A handful of similar medications have since been introduced, yet the little blue pill still remains a top seller. And it appears likely to remain a major factor in the U.S. market at least until December 11, 2017, which is the earliest date that a generic version of Viagra could go on sale in this country.
Pfizer’s Patent Good Till 2020
Pfizer’s U.S. patent on Viagra is good through April 2020, but a few years back, it struck an agreement with Israeli-based Teva Pharmaceuticals that would allow the latter to start selling a generic formulation in late 2017. However, because Pfizer will still hold the patent, Teva will need to pay the U.S. pharmaceutical giant a royalty for the license to produce its generic version of the little blue pill.

Sildenafil Citrate

Although they soon discovered that the compound was not all that effective as an angina treatment, their attention was caught by an unusual side effect. Male participants in the drug trials reported an increase in the number of erections they were experiencing. Among the participants were some men with a history of impotence. Pfizer soon discontinued its pursuit of the drug as an angina therapy and began to study the compound as a possible treatment for erectile dysfunction. The rest is history.
Viagra was the first in a family of closely related drugs known as PDE5 inhibitors, so-called because they temporarily disable an enzyme known as phosphodiesterase-5. The PDE5 enzyme can interfere with the erectile process, particularly in men whose blood flow to the penis is already compromised to some degree. Since Viagra’s approval in March 1998, the FDA has approved a handful of similar drugs, including Levitra (vardenafil hydrochloride), Cialis (tadalafil), Staxyn, an oro-soluble formulation of vardenafil hydrochloride, and Stendra (avanafil).
It’s All about Blood Flow
Although medical professionals thought for years that the primary cause of erectile dysfunction was psychological in origin, it has since been shown that the main culprit is actually insufficient blood flow to the penis. To understand how Viagra and the other PDE5 inhibitors work, it’s helpful to know a bit about the erectile process itself.
The initial impetus for an erection originates in the brain as a response to feelings of sexual desire or a reaction to physical stimuli of the genitals. Once the brain detects such stimuli, it sends a flood of nitric oxide coursing toward the pelvic region. The nitric oxide triggers secondary chemical reactions, one of which gives rise to a compound known as cyclic guanosine monophosphate, or cGMP. This cGMP compound causes the smooth muscles lining the arteries that supply the penis to relax, thus increasing blood flow to the organ.

And it’s at this point that the ability of Viagra and its close relatives to disable the PDE5 enzyme comes into play. It turns out that one of PDE5’s primary tasks in life is to break down cGMP. For normal healthy men with no significant blood-flow problems, this isn’t much of a problem, as the body produces enough cGMP to last throughout the erectile process. However, for those with vascular problems, the rampant PDE5 enzymes can pretty much shut things down, making it either difficult to get and/or keep an erection.
A Window for Sexual Activity
Viagra and the other closely related ED drugs hold the PDE5 enzymes at bay for three to four hours, making it possible to achieve an erection, enjoy sexual activity, and relax before the PDE5 enzymes return in full force to do their job of breaking down cGMP.
Clinical tests have shown that Viagra is effective and safe for most men. However, its use is not recommended for men whose doctors have advised them that sexual activity could endanger their health. Nor should it be used by men who are taking nitrate-based drugs. Both Viagra and nitrates work their magic by significantly lowering blood pressure. Taking them together can cause such a precipitous drop in blood pressure as to be life-threatening.
Common Side Effects
Viagra’s most common side effects are mild and often disappear upon continued use of the drug. Such adverse effects include back and muscle pain, dizziness, flushing, headache, nausea, rash, runny or stuffy nose, and upset stomach. Although rare, Viagra’s more serious side effects include an erection that lasts four or more hours and a sudden loss of hearing or vision.
