Most men who take tadalafil do fine. A headache the first day, maybe some flushing, and then the medication does what it’s supposed to do. But tadalafil side effects range from the mildly annoying to the genuinely dangerous, and knowing the difference is what keeps a useful medication from becoming an ER visit.

Tadalafil (the generic version of Cialis) is FDA-approved for erectile dysfunction, benign prostatic hyperplasia, and pulmonary arterial hypertension. It works longer than sildenafil, which is why some patients prefer it. Longer half-life also means side effects can linger longer if they show up.

What follows are the five warning signs that should prompt a phone call, not a wait-and-see. Most patients will never see any of them. But you should know what you’re looking for.

How Tadalafil Actually Works (and Why Side Effects Happen)

Tadalafil is a PDE5 inhibitor. It relaxes smooth muscle and increases blood flow, and not just to the penis. The same mechanism affects blood vessels throughout your body, which is why the most common tadalafil side effects show up as headache, flushing, nasal congestion, back pain, and muscle aches.

According to the National Library of Medicine, roughly 1 in 10 men taking tadalafil report headache, and about 1 in 20 report indigestion or back pain. Most of these fade within a few days as the body adjusts.

The trouble starts when a side effect stops being a nuisance and starts being a signal that something’s wrong. That’s what these five warnings are about.

5 Warning Signs to Take Seriously

1. Chest Pain, Pressure, or Pain Radiating to Your Arm or Jaw

This is the one nobody wants to talk about but everybody needs to know. Tadalafil dilates blood vessels, which lowers blood pressure. If your heart isn’t up to the demands of sexual activity plus a vasodilator on board, you can end up with angina or worse.

If you develop chest pain during or after taking tadalafil, do not take nitroglycerin. The combination of nitrates and PDE5 inhibitors can drop blood pressure to fatal levels. Call 911 instead. 2. An Erection Lasting More Than 4 Hours (Priapism)

Priapism is rare but a genuine emergency. Blood gets in, blood can’t get out, and after about four hours the tissue starts to suffer. Permanent damage to erectile function is possible if this isn’t treated quickly.

The Cleveland Clinic notes that priapism from PDE5 inhibitors is uncommon but higher in men with sickle cell disease, leukemia, or multiple myeloma. If you get an erection that won’t quit, don’t tough it out. Go to the emergency room.

3. Sudden Vision Changes or Vision Loss in One Eye

There’s a rare condition called non-arteritic anterior ischemic optic neuropathy (NAION) associated with PDE5 inhibitors. Blood flow to the optic nerve drops, and vision in one eye can be permanently lost. Risk is higher in men over 50, smokers, and those with diabetes or high blood pressure, which describes a lot of the tadalafil-using population.

If your vision suddenly blurs, dims, or goes dark in one eye, stop taking tadalafil and get evaluated the same day. Blue-tinted vision (which is more common with sildenafil, per our Viagra side effects guide) is usually harmless and temporary. Sudden vision loss is not.

4. Sudden Hearing Loss or Ringing in the Ears

Less well-known but real. The FDA added a warning about sudden sensorineural hearing loss with PDE5 inhibitors back in 2007. It’s usually unilateral, often accompanied by tinnitus or dizziness, and can be permanent if not caught early.

If your hearing changes suddenly after a dose, stop the medication and see a doctor. Steroids given within a few days can sometimes reverse it.

5. Severe Dizziness or Fainting

Tadalafil lowers blood pressure. If you’re also on an alpha-blocker for BPH (like tamsulosin), or on multiple antihypertensives, the combined effect can drop your pressure enough to make you pass out. Standing up too fast is when it usually hits.

Who’s at Higher Risk for Tadalafil Side Effects

Certain groups need extra caution:

  • Men with cardiovascular disease, especially recent heart attack or stroke
  • Anyone on nitrates (nitroglycerin, isosorbide) in any form
  • Men with severe liver or kidney impairment (dose adjustments needed)
  • Men over 65, who metabolize the drug more slowly
  • Anyone on strong CYP3A4 inhibitors like ketoconazole or ritonavir

Your prescriber will determine whether tadalafil is appropriate and at what dose. Standard starting doses range from 2.5 mg daily to 20 mg as-needed, but that decision depends on your kidney function, other medications, and what you’re treating.
