Men suffering from symptoms of infertility might be heartened to learn that oral ED drugs appear to improve the quality and movement of sperm.

Preliminary evidence from a review of relevant scientific literature appears to show that PDE5 inhibitors — oral medications such as Viagra and Cialis prescribed to treat erectile dysfunction — can improve sperm quality and motility.

Could these oral ED drugs offer yet another way to treat male infertility? It may be too early to tell.

Although the results from this small-scale review are promising, other researchers caution that more research needs to be done before making any concrete recommendations about the benefits of PDE5 inhibitors on male fertility.

Such concerns are detailed in a chapter of 2018’s “Bioenvironmental Issues Affecting Men’s Reproductive and Sexual Health” that was written by a team of urologists and andrologists from Greek and Japanese academic centers.

Benefits Enumerated

In the latter publication, the scientists tick off a number of findings that tend to show PDE5 inhibitors’ benefits on male reproductive health. These include the apparent ability of PDE5 inhibitors to stimulate the Leydig cells that secrete testosterone and also play a role in regulating the contractility of the tunica albuginea and the epididymis, key players in sperm production. The researchers also point to findings that the ED drugs increase the prostatic secretory function, thus leading to improvement in sperm motility.

Despite these promising findings, the authors conclude that additional studies must be completed before it can be confirmed that there is a therapeutic role for PDE5 inhibitors in the treatment of male infertility. They also warn that “a growing abuse of this group of drugs raises concerns about the potential risks to reproductive health.”

Yet Another Study Shows PDE5 Benefits

Some of the more promising evidence of PDE5 inhibitors’ beneficial effects on male fertility come from a Chinese analysis of data from 11 recent relevant scientific studies in which a total of 1,317 participants were involved. The results of that review were published in the July 2017 issue of “Urology.”

Evaluating the data from these studies, the Chinese research team found that administration of PDE5 inhibiting drugs appeared to have little if any, the effect on the volume of semen or the concentration of sperm within the seminal fluid.

However, it did conclude that infertile men taking PDE5 inhibitors had a noticeable increase in the motility of their sperm and also showed an increase in their number of normally shaped sperm. Similar improvements were not observed in men who were fertile.

The Chinese researchers noted that improvements were most impressive among infertile men who took PDE5 inhibitors on a regular basis rather than only once in a great while.

They concluded that regular use of these ED drugs “could modestly increase sperm motility and morphology [shape] in infertile men.”
