A Turkish study says that low-dose daily tadalafil appears to be the most appropriate form of dosing for the drug among diabetic men over the age of 65. For men under 65, both daily and on-demand tadalafil seem equally effective.
Among users of Cialis and its generic equivalent, tadalafil, the debate continues: Which is best: daily therapy or long-lasting on-demand tablets?
For the vast majority of men, it’s largely a question of personal preference. Do they prefer the spontaneity offered by taking low-dose Cialis or tadalafil on a daily basis or the long-lasting effects of the drug at the higher dosage levels contained in its on-demand formulation? Low Dose Best for Some Men
However, a study published in the summer of 2018 suggests that the low-dose daily formulation might be preferable for certain groups of men.
A team of Turkish urological researchers recruited a study group of 63 male diabetics, all of whom suffered from erectile dysfunction. The goal of the team’s study was to determine which of the two tadalafil protocols — daily or on-demand — was most beneficial and to gauge these protocols’ effects on ejaculatory function and lower urinary tract symptoms (LUTS).
The study participants were divided into two groups. The 31 study participants in the first group were given 5-milligram tablets of tadalafil daily for three months. The 32 test subjects in the second group received four monthly doses of 20-milligram tadalafil over the same three-month period.
Results of the Study
Men under 65 in both groups showed improved scores on the International Index of Erectile Function (IIEF), while those over 65 showed no real benefit in terms of erectile function. And both groups showed improvement in terms of erection quality and LUTS.
However, men who took the daily dose of 5-milligram tadalafil appeared to exhibit greater improvement in quality of ejaculation and LUTS than those taking the on-demand dose of tadalafil.
The Turkish researchers’ findings led them to conclude that “It may be beneficial to give 5 mg tadalafil daily to patients over 65 years old who do not benefit from treatment with 20 mg of tadalafil” or in patients over 65 who have LUTS. Their findings were published in the June 2018 issue of the International Journal of Impotence Research.
Contradicts Earlier Study
The Turkish study’s conclusions stand in contrast to the findings of an earlier study comparing the benefits of daily vs. on-demand tadalafil. The latter study, published in a 2016 issue of The Journal of Sexual Medicine, found that “No clinical populations of patients with ED seemed to benefit overwhelmingly from one dose regimen over the other.”
