The path from drug manufacturer to consumer consists of three legs:<\/p>\n<ul>\n<li>The transaction between the drug manufacturer and the wholesaler<\/li>\n<li>The transaction between the wholesaler and pharmacy<\/li>\n<li>The transaction between the pharmacy and consumer<\/li>\n<\/ul>\n<p>The &#8220;usual and customary&#8221; price of a drug reflects the cost of a drug to a consumer without the use of insurance covering prescription drug costs. It is also sometimes referred to as the &#8220;cash price.&#8221;<\/p>\n<p>At each leg of a drug&#8217;s journey from manufacturer to consumer, many factors are at play. From manufacturer to wholesaler, there may be rebates or discounts. From wholesaler to pharmacy, discounts and rebates may affect the pharmacy&#8217;s cost of a given drug. From pharmacy to consumer, a number of factors affect the amount the consumer pays.<\/p>\n<p><strong>Factors Influencing What You Actually Pay at the Pharmacy<\/strong><\/p>\n<figure id=\"attachment_7576\" aria-describedby=\"caption-attachment-7576\" style=\"width: 300px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-7576\" src=\"http:\/\/blog.edrugstore.com\/wp-content\/uploads\/2012\/11\/indy-pharmacy-300x232.jpg\" alt=\"\" width=\"300\" height=\"232\" \/><figcaption id=\"caption-attachment-7576\" class=\"wp-caption-text\">Independent pharmacies are often competitive on prices of name brand drugs.<\/figcaption><\/figure>\n<p>What you pay for a prescription at your pharmacy varies based on several factors:<\/p>\n<ul>\n<li>Whether you have prescription drug coverage under your insurance<\/li>\n<li>Whether you are purchasing a single-source, name brand drug for which there are no name-brand or generic substitutes<\/li>\n<li>Whether generic alternatives exist<\/li>\n<li>Pharmacy mark-up<\/li>\n<\/ul>\n<p>While people with prescription drug coverage are shielded from some of the biggest variations in drug costs, those who do not have prescription drug coverage may find a huge range of prices that depend largely on pharmacy chain discount programs and whether the drug is name brand or generic.<\/p>\n<p><strong>Price Variation with Name Brand, Single Source Drugs<\/strong><\/p>\n<p>With name brand, single source drugs, the discounts to consumers come almost entirely from prescription drug coverage. In other words, if you are taking the only drug appropriate for your condition, if there is no generic, and if you do not have prescription drug coverage, you will likely pay a similar price whether you buy at your local independent pharmacy or at a big chain pharmacy. In these situations, even large chains have limited negotiating power.<\/p>\n<p><strong>Price Variation with Name Brand, Multi Source Drugs<\/strong><\/p>\n<p>If there is no generic alternative to treat your condition and you have no prescription drug coverage, but there are multiple name brand drugs that may be appropriate, you may be able to work with your physician to find the name brand drug that will have the lowest cost to you. There is some trial and error involved in this, because the drug that works best for you may end up being the more expensive one. But if you have a condition for which there are several name brand drugs, you may get good results from one of the less expensive choices.<\/p>\n<p><strong>Price Variation with Generic Drugs<\/strong><\/p>\n<figure id=\"attachment_7577\" aria-describedby=\"caption-attachment-7577\" style=\"width: 300px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-7577\" src=\"http:\/\/blog.edrugstore.com\/wp-content\/uploads\/2012\/11\/rite-aid-sign-300x196.jpg\" alt=\"\" width=\"300\" height=\"196\" \/><figcaption id=\"caption-attachment-7577\" class=\"wp-caption-text\">When you&#8217;re prescribed generics, it&#8217;s definitely worth your while to call around and compare prices.<\/figcaption><\/figure>\n<p>Generic drug prices generally have the widest range of prices. The reason is that pharmacies have more negotiating power with generic drug manufacturers than with name brand drug manufacturers. Additionally, pharmacy mark-up varies greatly with generic drugs. Pharmacies that take a loss or have razor-thin profits on name brand drugs may try to make up for those lost profits by charging high prices for generics. The only way to make sure you&#8217;re getting the lowest price for a generic is to ask and compare prices among several pharmacies.<\/p>\n<p><strong>Prices of Three Popular Name Brand Drugs by State<\/strong><\/p>\n<p>Three of the most-prescribed drugs in the United States are Lipitor, Lexapro, and Singulair. Lipitor is a &#8220;statin&#8221; drug that slows production of cholesterol to decrease build-up of cholesterol plaques on artery walls and reduce the risk of heart attack. Lexapro is an antidepressant that works by decreasing reuptake of serotonin in the brain. Singulair is a drug used to treat asthma and seasonal allergies. We checked prices of these three name brand drugs in pharmacies in all 50 states. The prices of these name brand drugs vary minimally from state to state and between chain and independent pharmacies. The following table gives representative prices of Lipitor, Lexapro, and Singulair in each state for a supply of 30 tablets of the standard dosage (which is coincidentally 10 mg for each of
