R<\/span>esearch has shown that rehabilitation therapy with tadalafil, the active ingredient in Cialis, can minimize or eliminate reduction in penile length after a prostatectomy.<\/em><\/strong><\/p>\n

For many men, a diagnosis of prostate cancer carries with it not just the specter of a potentially life-threatening illness but also a very real threat that surgical procedures to remove cancerous tissue could put a premature end to the patient\u2019s sex life.<\/p>\n

Although new surgical techniques hold the promise of reducing the incidence of postoperative sexual dysfunction, some of these may result in a reduction of penile length. Although such reductions are generally slight, any reduction in penis length — no matter how small — is an unwelcome prospect for most men.<\/p>\n

Nerve-Sparing Prostatectomy<\/h3>\n

Among the newer of the surgical techniques for dealing with prostate cancer is the nerve-sparing radical prostatectomy, which is indicated for the removal of localized prostate cancer. This type of surgery can reduce penile length and almost always results in a period of sexual dysfunction.<\/p>\n

However, a study published in the May 2014 issue of \u201cBJU International\u201d offers evidence that tadalafil (the active ingredient in Cialis) rehabilitation therapy can minimize reductions in penile length after radical prostatectomy. And that wasn\u2019t the only good news from this study conducted by researchers at Memorial Sloan-Kettering Cancer Center in New York.<\/p>\n

Other Benefits Noted<\/h3>\n

In a presentation before a meeting of the American Urological Association in Orlando, researchers said that patients treated daily with 5 milligrams of tadalafil also showed improvements in the ability to achieve erections suitable for intercourse. Also noted was an increase in the incidence of morning erections among postsurgical patients.<\/p>\n

In the Sloan-Kettering study, patients treated with tadalafil had an average of 4.1 millimeters less reduction in stretched penile length when evaluated nine months after surgery, compared with patients who were treated with placebo. For those unfamiliar with the metric system, 4.1 millimeters amounts to just over 0.16 inch. It may not seem like\u00a0much if it\u2019s coming off the other guy\u2019s penis, but it’s probably more than you would care to lose.<\/p>\n

Minimizes Penile Length Loss<\/h3>\n

Commenting on the implications of the Sloan-Kettering findings, Gerald Brock, M.D., professor of urologic surgery at the University of Western Ontario, said: \u201cThese data suggest that tadalafil at a dose of 5 milligrams daily, if started early after surgery, protects against penile length loss and may help prevent structural cavernosa changes after nerve-sparing radical prostatectomy.\u201d The penis has two corpora cavernosa, which are sponge-like cylinders of erectile tissue that fill with blood upon sexual arousal.<\/p>\n

The Sloan-Kettering study is good news for men who may have to undergo nerve-sparing radical prostatectomy, because it indicates that the tadalafil rehabilitation therapy not only preserves penile length but also speeds the return of normal sexual function.<\/p>\n

According to OncLive<\/a>, sexual dysfunction is a common occurrence after radical prostatectomy. However, the majority of men who had normal erections prior to surgery recover function eventually, although full recovery make take several years. The use of tadalafil therapy holds out the promise that the postoperative recovery period can be significantly shortened.<\/p>\n

Assembled for the Sloan-Kettering research was a study group of 423 men, all of whom had normal sexual function before having radical prostatectomy. After surgery, one group received low doses of tadalafil daily, the second got varying doses of tadalafil on demand, and the third received a placebo.<\/p>\n

Only a 2.2-Millimeter Reduction<\/h3>\n

The stretched penile length of each patient was measured before surgery and at varying intervals after the surgery. Researchers found that patients on the low dose of tadalafil had an average of 2.2 millimeters in penile reduction, compared with an average of 6.3 millimeters lost in the patients getting placebo. A reduction of 2.2 millimeters amounts to a drop in penile length of about .09 inch, but it\u2019s significantly less than the quarter-inch loss experienced by patients in the control group.<\/p>\n

Interviewed about other aspects of their postoperative sexual function, significantly more patients in both of the tadalafil groups answered affirmatively regarding their ability to achieve at least some degree of erection at 9 and 13 1\/2 months, compared with those in the control group.<\/p>\n

Maximizes Blood Flow<\/h3>\n

Cialis and the other impotence drugs currently on the market work their magic by maximizing blood flow to the penis, which is essential to achieve and maintain erection. Cialis and the other ED drugs belong to a family of medications known as phosphodiesterase-5 inhibitors, or PDE5 for short.<\/p>\n

To achieve a normal erection, the chambers of the penis must fill with blood, a process that occurs when blood vessels carrying blood to the penis expand and the blood vessels that carry blood away from the penis contract. Temporarily engorged with blood, the penis remains erect.<\/p>\n

As part of this complicated process, a man\u2019s feelings of sexual arousal cause his body to send a flood of nitric oxide to his penis, thus triggering a series of chemical reactions necessary to achieve erection. Nitric oxide signals an enzyme known as guanylate cyclase to synthesize a byproduct known as cyclic guanosine monophosphate, or cGMP, which controls the expansion and contraction of blood vessels to and from the penis.<\/p>\n

PDE5 Enzyme Can Destroy cGMP<\/h3>\n

The PDE5 enzyme is a natural enemy of cGMP and will destroy it if possible. Because Cialis and the other PDE5 inhibitors temporarily block the effects of PDE5, erection can more readily occur and be sustained long enough for the completion of sexual activity.<\/p>\n
